Rather as I predicted, AP went ahead with his heart surgery and ran into some difficulties.  He ended up with a triple bypass, a TAVR (Transcatheter Aortic Valve Replacement), and a mitral valve replacement.  Coded once, took a long time to come out from sedation, and is already having BP issues and excessive bleeding from the vein graft.  Recovery will be measured in months (if it happens).

Of course, the best part was the discussion with the surgeon after the procedure.  He actually said to my cousin (the wife of the patient), "Well, he may not see any significant improvement to symptoms and quality of life."  Some words to the effect of mentioning that before the surgery were exchanged, of course.
